and print, thereby saving time when you arrive at the Spa.
These forms are:
General Client Intake Form:

This form is for all guest to complete. It contains a basic
health questionnaire as well as specific contact information in the
event of an emergency.
Client Intake Form - Body Wraps and
Facials:

This form, used in conjunction with the General Intake Form,
provides specific information regarding the cleansing products you
currently use, prescription medicines you are taking that might
react with the treatments we provide, and general skin tone/quality
information.
Client Intake Form -
Massage:

This form, used in conjunction with the General Intake
Form, provides specific information regarding muscle tension, joint
pain, and other factors that might affect your
massage.
Please bring these forms with you!
After you complete the General Intake Form and other form(s) as
applicable, please bring them with you to your first appointment.
This will allow you more time to relax in our Decompression room
and enjoy the other amenities of the Spa.
